<div><p>An exciting opportunity has arisen for an experienced and motivated<b>Community Nurse</b> to join our Care Home Team as a <b>Band 6 Deputy Team Lead</b>.</p><p>Based at <b>Stanmore (HA7 1AT)</b>, the Care Home Team delivers high quality nursing care to residents across care homes within <b>Hertsmere, St Albans and Harpenden, Dacorum, and Watford</b>.</p><p>You will be supported by an experienced Band 7 Team Lead, Practice Development Nurses, and a range of specialist link nurses, with access to excellent learning and development opportunities through the CLCH Academy.</p><p>This role is ideal for an experienced community nurse looking to develop their leadership skills within a supportive and forward thinking team. The post holder will be required to work across the locality in response to service needs, including participation in a 1 in 4 weekend rota. As travel between care homes and community settings is required, a full UK driving licence and access to a vehicle for work purposes are essential.</p><p>If you are passionate about delivering outstanding care, developing others, and progressing your leadership career, we would love to hear from you.</p><h3>Main duties of the job</h3><ul><li>To set and maintain standards of care and deploy, co-ordinate and supervise the nursing team and learners as required.</li><li>As a Deputy Team Lead, you will be responsible for managing a caseload of patients within residential care settings, providing clinical leadership and supporting the day to day management of the team.</li><li>You will line manage and support <b>Band 5, 4, and 3 </b>staff, helping to develop confident, autonomous practitioners whilst ensuring the delivery of safe, effective, and compassionate care.</li><li>To provide leadership and management to a team of community nurses and other staff covering the four localities across Herts Valleys.</li><li>To be responsible for the teams assessment, planning, implementation and evaluation of individualised nursing care to patients living in a residential home. The service is provided seven days per week 8pm-8am.</li><li>Tobe responsible for the teams maintenance of patient and other records.</li><li>Toparticipate in the long term management of patients within the locality.</li><li>To have continued responsibility for a defined caseload.</li></ul><h3>Job responsibilities</h3><p>Successful applicants will be required to demonstrate alignment with the Trusts values & the behaviours aligned to those values - Accountability, Inclusion, Compassion, and Empowerment, alongside fulfilling the duties detailed in the attached Job Description and Person Specification.</p><h3>Person Specification</h3><h3>Ediucation/Qualifications</h3><ul><li>Registered Nurse</li><li>Practice Supervisor/Assessor or equivalent</li><li>Specialist Nursing Practice(District Nursing) Degree or will undertake training</li><li>Nurse Prescribing or will undertake training</li><li>Knowledge of recent developments in NHS</li><li>Knowledge of Evidence based practice</li><li>Evidence of personal development</li><li>Knowledge of NMC code of professional conduct</li><li>Relevant courses in Long Term Conditions Management</li><li>Course of Top to Toe examination/ diagnosis Experience</li></ul><h3>Experience</h3><ul><li>Experienced Nurse</li><li>Management of a team</li><li>Management of change within a complex environment</li><li>Experience of teaching others</li><li>Proven clinical skill</li><li>Experience of participating in investigation/ management of complaints</li><li>Evidence of participating in clinical audits in the workplace</li></ul><h3>Skills and Knwledge</h3><ul><li>Management of change</li><li>Knowledge of negotiation skills</li><li>Understanding of current political and policy context in Primary Health Care</li><li>Knowledge of clinical governance and its implications</li><li>Evidence of expertise in clinical skills</li><li>Experience of teaching others</li><li>Knowledge of research processes</li><li>Knowledge of resource management</li><li>Knowledge and understanding of Adult protection issues</li><li>A understanding of working within a competency framework</li></ul><h3>Other</h3><ul><li>Car driver and holder of current UK driving licence (unless you have a disability as defined by Disability Discrimination Act 1995)</li></ul><h3>Disclosure and Barring Service Check</h3><p>This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.</p><p>£45,953 to £54,254 a yearper annum, inclusive of HCAS</p></div>
