District Nurse Planned Care Nursing East/West

Are you an experienced Nurse with a District Nursing qualification (DN SPQ)

Where you will be working

* You will have the opportunity to work in one of our Primary Care Network (PCN) Teams covering Bexhill, Hastings / St Leonards and Rural Rother. and Eastbourne, Hailsham & Seaford
* You would need to have access to a car and drivers licence for this community role.

Main duties of the job

What you will be doing

* A District Nursing qualification (DN SPQ) is an essential criteria.
* Assessing new patients for nursing care in their own homes, creating and evaluating care plans.
* Managing a caseload with support of the Clinical Lead
* Line Management and leadership of a team of Registered Nurses & HCAs.
* Our core service hours are 08:00 to 18:00, 7 days per week.

Person Specification


Qualifications

* Community Specialist Practitioner Qualification - District Nursing
* RN part 1 on NMC register
* Nurse prescriber / V100
* NMC required teaching & assessing module, or commitment to undertake


Experience

* Teaching in a clinical setting
* Experience of supervising a team
* Experience of caseload management
* Experience of multidisciplinary working
* Experience of managing change and developing teams

£37,338 to £44,962 a yearper annum, pro rata
