Belmont Recruitment are currently seeking an experienced Leaving Care Worker to work with a North East Local Authority on a temporary basis. This is a full-time role working 37 hours per week.
Main Duties
* Facilitate assessments of need and ongoing monitoring to support young people’s health, education, and wellbeing
* Provide practical, personal, and emotional support, including domestic tasks and transporting where necessary
* Advise on key areas such as health, benefits, education, training, employment, housing, and personal well-being
* Develop and implement Pathway Plans and Personal Action Plans with young people
* Advocate for young people and empower them to make decisions and self-advocate
* Work alongside colleagues and other agencies to ensure needs are met within agreed plans
* Maintain accurate records and write reports using electronic systems
* Support young people through major transitions, such as moving to adulthood
Essential Criteria
* NVQ Level 3 or equivalent (e.g., relevant degree, DipSW, RGN, Teaching, Youth and Community)
* Minimum of 2 years’ continuous experience working with young people (post-16)
* Knowledge of Children’s Services policies, procedures, and safeguarding regulations
* Ability to record accurate information and write reports using electronic systems
* Effective communication and engagement skills with children, young people, and families
* Understanding of child and young person development, including assessing developmental stages
* Knowledge of safeguarding, risk assessment, and local procedures
* Experience supporting transitions and working collaboratively with other professionals
* Full driving licence and access to a vehicle (or driver) for work purposes
* Flexibility to work outside normal working hours as required
