Were looking for a proactive, compassionate professional to join our multi-disciplinary discharge team, supporting vulnerable residents with complex needs as they leave hospital. Youll work closely with NHS colleagues, housing providers, and community services to ensure safe, timely discharges and prevent homelessness.As a vital link between the hospital and community settings, youll assess needs, coordinate personalised support, and help residents stay independent while reducing repeat admissions. Main duties of the job

To provide professional support to some of the mostvulnerable people in the Borough. You will be working as part of a busymulti-disciplinary discharge service which is outcome focussed and creative inachieving successful outcomes for our residents, offering personalisedsolutions on a day-to-day basis.

Working collaboratively with other agencies andorganisations you will help facilitate discharges from health care settings,helping people to access local preventative support services and to avoidrepeat admissions to acute health care services.

You will be an integral part of a busy team andprovide an essential link between the hospital and the community. Working withlocal authority and health service partners you will carry out assessment forvulnerable residents with complex needs, connecting them with support in theircommunity which will help them to avoid further crisis, prevent the loss ofaccommodation and continue living independently. This will also help avoid repeat admissionsand reduce demand on statutory services.

You will work from various locations alongside otherprofessionals and duties will involve follow up visits to residents in thecommunity. You will be confident workingalongside other professionals across housing, health and social care whilemaintaining a customer focus to achieve successful outcomes.

Job responsibilities

To work collaboratively with colleagues, partneragencies and voluntary sector and contribute to the effective delivery of theintegrated preventative service across housing, health and social care toachieve successful outcomes and reduce the need for statutory interventions.

Adopt a specialist lead role for Slough residentssupporting people complex lives to be safely discharged from local hospital andhealthcare settings and support back into the community.

Work flexibly to meet service needs and the needs ofresidents, providing practical, support and assistance to enable safe andtimely discharges from hospital to appropriate accommodation.

Ensure community-based support is in place to preventrepeat and inappropriate admissions to acute health care services andfacilitate access to lower-level preventative services.

Work in a person-centred way, carrying out holisticassessments and investigations to identify housing, health, social care,financial and employment needs and work in collaboration with other agencies toensure those needs are met.

Coach and empower individuals to increase theirability to gain and maintain their independence.

Think creatively and decisively to find and implementbespoke solutions across services who have conflicting agendas to prevent andresolve homelessness and prevent escalation of need.

Devise, monitor and review holistic personalised plansto achieve successful housing, health and social care preventative outcomes.

Adopt a casework-based approach while working withcompeting priorities in a changeable environment, ensuring targets and serviceobjectives are met.

Record, monitor and track clients, ensuring relevantsystems are kept up to date with concise, accurate, evidence-based records,identifying and recording risk issues to minimise risks to themselves and toothers.

Keep up to date with key legislation, policies andprocedures, in the areas of homelessness and eligibility for housing andhomelessness assistance, and the councils statutory duties in relation tohousing, health and care needs, to ensure high quality, timely and accuratedecision making.

Maintain up to date service information andproactively build professional relationships with other professionals, makingreferrals to internal departments, external agencies and other localauthorities and proactively following up to ensure referrals are acted on.

Cover for other staff across the service as requiredand undertake other duties commensurate with the purpose of the post.


Person Specification


Qualifications

* Evidence of Continuous professional development
* Full valid driving licence and use of a car with business usage insurance
* Additional recognised management qualification


Experience

* Relevant work-based experience of working in an operational team in a hospital, housing or Adult Social Care setting
* Experience in organising and managing a varied and complex caseload
* Experience of working with people experiencing homelessness
* Experience of working collaboratively across multi-professional teams and other partners organisations
* Experience in managing and applying process procedures to ensure proper Governance and compliance.
* Experience of working with people with substance and alcohol use.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

£41,873 to £47,238 a yearinclusive of £1,096 Local Weighting Allowance of per annum.
