What you will do:
* Collaborate with CAMHS, children's services, and adult mental health teams to create and implement smooth transition plans.
* Advocate for young people's needs by developing personalised care plans to promote their independence and well-being.
* Engage with families and carers to support their involvement in the transition process.
* Conduct comprehensive assessments to identify support needs and help develop community engagement and independent living skills.
* Work within Care Act principles to ensure young people can gain employment and live independently.
* Manage a varied caseload, maintaining up-to-date records of interventions and outcomes.

* An experienced and qualified Social Worker registration to practice with Social Work England.
* A current full driving licence and access to a vehicle for travel across Devon.
