Description Social Worker | 9840 | 12 month Fixed Term Contract | 37 hours per week | £39,862 to £41,771 per annum (Scale J) | Norwich Community Hospital An exciting opportunity has arisen for a Social Worker to join our dedicated team supporting adults during their recovery and reablement journey following a hospital stay. This is a 12 month fixed term contract based at the Norwich Community Hospital, supporting discharge from a recently opened Reablement unit. As part of a small, dynamic team, you will work closely with individuals in intermediate care beds, helping them regain independence and transition back to their own homes. Our service follows a Home First ethos, ensuring person-centred care through strong collaborative working with a multi-agency team of health and social care professionals. This is a hybrid role offering great opportunities for multi- disciplinary working with a wide MDT, the candidate will need a good knowledge of Social Care legislation and assessments.
