Advance Nurse Practitioner - Band 7
The successful post holder will be responsible for the assessment of patients who become acutely unwell in the community to ascertain their medical needs and formulate a clinical diagnosis for prompt treatment thereby preventing avoidable hospital admission.
Main duties of the job
To practice case management model of delivery for specialist care including Long Term Conditions and care in the period of exacerbation by assessing, planing, prioritising, implementing and evaluating appropriate interventions to meet patients' needs which may include prescribing pharmacological agents to patients.
Undertake comprehensive health care needs assessment of patients, reassessing as appropriate, working flexibly between the acute hospital and community locations.

Qualifications
Essential criteria
1. RGN BSc/MSc /Dip/Degree
2. V-300 Independent prescribing qualification.
3. Advanced physical assessment. qualification.
4. Managing Minor illness.
Desirable criteria
5. History taking
6. Abilities to recognize and manage deteriorating patients.
Skills
Essential criteria
7. Intravenous antibiotics, Cannulations,, Phlebotomy, Syringe Driver.
8. Ability to manage and support a team of Nurses and a caseload of patients.
9. Ability to liaise effectively with members of the Primary Health Care Team and other agencies.
10. Good organisational skills and ability to prioritise and work in a self-directed way
11. Male, Female suprapubic Cauterization
Desirable criteria
12. managing patient flow in the hospital settings.
13. Qualification in district or other relevant community settings.
Knowledge
Essential criteria
14. Understanding and knowledge of the complex needs of individuals and families in a deprived, urban area
15. Knowledge of agencies/organisations which can be utilised for the benefit of patients
16. Knowledge of current professional issues facing urgent community response services.
Desirable criteria
17. Car Driver Current UK valid driving license
18. Experience of working in similar settings.
