A Support Coordinator (Hospital Discharge) will support homeless or rough sleeping customers to reduce length of stay in inpatient wards, create person‑centred discharge plans, and partner with hospitals, housing and other services to facilitate smooth transition to accommodation.
What you’ll do
* Visit wards at Musgrove Park Hospital (one day per fortnight) and community hub, The Link in Yeovil.
* Partner with local hospitals and housing teams to reduce average stay on inpatient wards for customers with complex needs.
* Create and review person‑centred discharge plans with customers.
* Collaborate with professionals in health, housing and other relevant services to ensure a smooth transition from hospital to accommodation.
You have
* Passion to support customers and work collaboratively with an eye for detail.
* Experience creating person‑centred support plans and working with colleagues to implement them.
* Knowledge of housing law and legislation.
* Experience coordinating and assessing customer referrals.
* The ability to work independently, remain calm under pressure and exhibit a resilient approach.
Practical bits
Working days: Monday to Friday 9am-5pm; may need to adapt start or finish times on occasion.
Capability to use technology for creating and updating support plans, complete online learning, and collaborate with colleagues.
Enhanced DBS check required – employer will pay for it; a vehicle insured for business use and a valid driving licence required (employer reimburses business mileage).
