An exciting opportunity has arisen for a suitablyqualified Social Worker, CPN or OT, at band 5 level, to work in the SouthKirklees Core Team: a well-established Community Mental Health Team for adults.The post offers a wonderfully diverse working role encompassing workingalongside other professionals within the Core team and seeking to createsupportive and recovery focused care plans with service users. The Core Team, in conjunction with the TraumaInformed Personality Disorder Pathway, aims to provide trauma-informed care toindividuals with complex needs. The post-holder will have the opportunity towork within a friendly, fully integrated multi-disciplinary team who are dedicatedto developing better services guided by trauma-informed principles of care.
Main duties of the job
Weare looking for an enthusiastic individual who is flexible and creative inresponse to service pressures and patient needs. The role will involve a widerange of activities including holding a caseload, working alongside otherprofessionals such as Psychiatry, OT, Psychological therapists and SocialWorkers to co produce recovery focused care plans. The Core Team offer a rangeof short term interventions such as, helping service users to gain anunderstanding of emotional regulations, dealingwith distress, crisis and contingency planning as well as identifying potentialOT interventions or addressing social care issues, including Care ActAssessments. The Core Team also support service users with more chronic mentalhealth needs and run Community Clinics and a Treatment Team offeringmedications and advice from our Psychiatrists. The Core Team also work withother local community organisations, such as the Recovery College, to aid asupportive discharge from secondary mental health services into primary care,including GPs.
Youwill be expected to work with a caseload and be able to work autonomously andas part of a large team. Regular supervision is available as well as acommitment to personal and professional development.

Job responsibilities
JOB PURPOSE: The post holder will be responsible and accountable for the assessment, planning, implementation and evaluation of skilled interventions to address identified needs and risk for allocated clients on a defined caseload. Works within a multi-disciplinary team as a member of a
Core CMHT.
KEY RESULT AREAS:
1. To work as a member of an integrated multi-disciplinary team providing comprehensive assessment of clients and their carers, planning, implementing and evaluating skilled interventions to address identified needs.
2. To have knowledge of current issues and research relevant to the legislative framework and systems in which mental health care is delivered CPA, MHA (1983).
3. To contribute to service development within the clinical governance structure and policies and procedures of the Trust. Adhering to NHS guidance outlined by the relevant statutory registration bodies.

4. Assesses needs and identifies problems relevant to the care of people referred. Devises a plan of care in partnership with the person and carers. Implements the planned programme of care to ensure a high standard is achieved. Reviews the effectiveness of the care provided and where appropriate initiates any action.
5. To be competent in all aspects of care and risk management as defined within the framework of the care programme approach.
6. To work collaboratively with clients, carers and multiple other agencies to provide effective care to optimize and maintain clients mental health and overall functioning, ensuring needs are met.
