Montpelier has an exciting opportunity for a Paramedic Practitioner to join our expanding team on a 6-month fixed-term contract. Job Summary

The post holder will be an experienced clinician, who, acting within their professional boundaries will provide clinical assessment and management of patients within the practice.

The post holder will support the delivery of high-quality care and will undertake service development and improvement of the local unscheduled care system, working within the team, across practices and with partners.


Responsibilities

You will be expected to provide and maintain a high standard of care for patients in their own homes as well as providing support to the GPs and other members of the primary health care team. Supported by a skill mixed team, as an autonomous, accountable, Paramedic Practitioner, in the provision of a holistic approach for patients including assessment, diagnosis and treatment, to deliver quality patient services.

The following sets out the key role responsibilities for paramedics:

* Assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the primary care team.
* Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services.
* Perform specialist health checks and reviews, and interpret ECGs.
* Perform investigatory procedures as required, and undertake the collection of pathological specimens including intravenous blood samples, swabs etc.
* Support the delivery of anticipatory care plans and lead certain community services.
* Provide an alternative model to urgent and same day home visits for the network and undertake clinical audits.
* Communicate at all levels across organisations ensuring that an effective, patient centred service is delivered.
* Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.
* Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.
* Prescribe/issue medications as appropriate or be willing to undertake Non medical prescriber training, following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways.
* Enhance own performance through continuous professional development, imparting own knowledge and behaviours to meet the needs of the service.


Team Working

Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working.

Ensure clear understanding and utilisation of referral mechanisms within the practice.

Prioritise own workload and ensure effective time-management strategies are embedded in own practice.
