About the Role
You will be part of a community-based NHS trust delivering integrated care alongside GPs, mental health professionals, social workers, and voluntary services. The role supports patients across the Marsh Primary Care Network and works from 08:00 to 18:00 with rotational weekend working.
Responsibilities
* Provide wound management, end‑of‑life care, medicines administration and bladder and bowel care.
* Conduct home visits for patients requiring specialist nursing input.
* Deliver care in local clinics supporting the community.
* Work with GPs, care homes, Social Services, voluntary organisations and other partners to keep patients safe and independent at home.
Qualifications and Requirements
* Band 4 Nursing Associate.
* Certifications allowing the use of a full UK driving licence (or equivalent EU licence) as the role requires driving.
* Right to work in the United Kingdom. Applicants must provide documentary evidence of their right to work.
