 
        Job Description As a Support Worker, you will work alongside adults with mental health issues, dual diagnosis due to substance misuse, learning disabilities and some individuals that may have behaviours that challenge. Service users will be supported in their homes (24//7 service) and in the community to live the best life possible. You will empower individuals to have choices about their own lives, develop skills to work towards independent living whilst building strong and positive relationships. You will work as part of a team to provide a dynamic supported living service. You will work with clients to ensure they receive a person centred, appropriate and safe service in their own homes. Knowledge and understanding of mental health and the needs of this service user group Knowledge and understanding of the Mental Capacity Act 2005 and its core principles. Knowledge and understanding of safeguarding and its core principals in accordance with the Care Act 2014. Ability to be proactive and work independently and as part of a team Main Duties * To key work one or more individuals, meeting each person weekly in order to provide appropriate support to them; this may include providing counselling, advice and support to service users whilst maintaining respectful and professional boundaries always. * Support people with mental health difficulties and those with dual diagnosis due to substance misuse to the highest standard and in line with their support plan, cultural needs and wishes. * Support people to manage their own home, including supporting individuals with maintaining their tenancies and give support and guidance when individuals move home. * Support people with personal care if required, which may include toileting and bathing, in line with their support plan. * Support and encourage people to make choices and life decisions in line with the Mental Capacity Act requirements. * Assist and encourage people to engage in everyday home, social, leisure and work activities, according to the individual’s choice. * Support people with their health needs as required, e.g. taking medication and attending appointments. * Keep accurate and up to date records. * Support people with their finances, where necessary, in line with guidelines. * Promote health, safety and well-being of the people we support. Including maintaining health and safety standards /infection control through cleaning etc * Supporting individuals to make positive choices about eating well, taking exercise and maintaining good hygiene. * Safeguarding. People with mental health and substance misuse issues can be vulnerable to abuse or exploitation. As their support worker you’ll make sure their rights are upheld and they are being treated with consideration and respect. * There will be some important administration to do as well, such as dispensing and recording medicines for the people you support and maintaining accurate records and reports. Everything should be in line with current legislative and care standards. * Work positively with colleagues, families and external organisations and professionals. * To be familiar with and act upon all the policies and procedures. * Improve the support provided by discussing any new ideas. * Participate in learning and development opportunities as required. * Undertake any other duties as appropriate. * To provide advice, information and support to assist people to make informed choices and to teach people new skills and maximise independence. * To contribute towards, sign and work in accordance with all working practices and risk assessments as well as agreed external assessments written by other agencies. * To be directly responsible for ensuring that service records are accurately maintained in a professional and confidential manner. * To implement the Equal Opportunities Policy in all areas of work. * Preparing for, attend and participating in regular team meetings and one-to-one supervision sessions with your line manager as well as attending appropriate service user related meetings as directed by the Registered Manager. * To familiarise yourself with the systems and structures of statutory and voluntary bodies. * To represent professionally always. * To participate fully in training, as directed by the management. * Any other duties appropriate to the post of Support Worker, as directed by the management
