We have a new opportunity to work as a Reablement Response Care Worker.
For this role you must have care experience and be a driver with a full, clean driving licence and access to your own vehicle.
As a Reablement Response Care Worker, you will help individuals regain independence and confidence after experiencing a decline in their health following a hospital discharge.
You will be covering Chester and the surrounding areas (Ellesmere Port, Malpas, Blacon, etc.).
You will be responsible for providing short‑term care to help customers get back on their feet after hospital discharge. You will help customers rebuild their confidence in day‑to‑day tasks and allow them to return to a normal independent life.
You will also receive a driver’s enhancement of 50p per hour on top of your pay rate.
You need to be an experienced care/support worker who has the drive to work with individuals to develop tailored support plans that address their specific needs and goals.
