Job overview
The Pharmacy Homecare Team has recently expanded. We are looking for a pharmacist who would like to be part of a growing service delivering medicines to over 5000 patients in a variety of specialities.
This is a varied role where you will gain exposure to prescription screening as well as the governance and service improvement in pharmacy homecare. You will have the opportunity to handle queries from patients and homecare providers and use your clinical and product knowledge to solve problems. If you have a systematic approach with attention to detail, this role is for you! You will also maintain clinical pharmacy experience supporting the clinical team depending on experience and service needs.
Main duties of the job
· To provide pharmacy homecare service and clinical pharmacy service in accordance with Trust procedures and priorities set by the Advanced Level pharmacist-Homecare.
· To apply all appropriate clinical and governance arrangements as set out in the Royal Pharmaceutical Society (RPS) standards and recommendations in the Hackett Report for Homecare services.
· Work with MDT to review existing and develop new homecare service programmes and specifications.
· To work with Lead Clinical Pharmacists and outpatients and other Pharmacy staff to identify, plan and develop new opportunities for homecare services.
· To identify and manage risks in relation to medicines use according to policy/protocol.

Detailed job description and main responsibilities
· To deputise for the Advanced Level Pharmacist – Homecare in the delivery of Pharmacy Homecare Service.
· To liaise with Clinical Lead Pharmacists and Advanced level Pharmacist -Homecare, to develop and implement a Pharmacy Homecare Service policy and strategy.
· To provide a high profile role in the clinical provision and development of pharmaceutical services where appropriate to other medical specialty areas related to homecare.
· To act as a role model for all pharmacy staff and specifically for junior pharmacists and trainee pharmacists
· To promote effective clinical and homecare pharmacy services.
Person specification
Qualifications and Training
* Registered member of GPhC
* Completion of Foundation stage 1 of PG General Practice Pharmacy
* Demonstrable knowledge gained through broad spectrum of clinical pharmacy training and experience
* Training and accreditation as a Practice Supervisor or Educational Supervisor
* Registered with GPhC as an Independent Prescriber
Skills and Experience
* Understanding of patient safety issues relevant to pharmacy services
* Experience of homecare pharmacy service
Personal/Professional Attributes
* Systematic, methodical, attention to detail attributes
* Excellent communication skills with patients and other health care professionals
* Leadership/innovation and motivational skills
