Job Summary
We have a rare opportunity for 2 Senior Clinical Practitioner fixed‑term posts (12 months). One full‑time and one part‑time position to cover maternity leave. We welcome applications from Registered Nurses, Occupational Therapists, or Social Workers. The role supports and treats service users aged 65+ experiencing severe mental distress in their own homes.
Main Duties
The successful practitioner works within a band 6 Senior Clinical Practitioner role, undertaking crisis assessments and formulating plans to support service users to remain at home. When unsafe, you will act as a gatekeeper for admissions to the ward. Your responsibilities include working autonomously, sharing knowledge of caseloads with the multidisciplinary team, and demonstrating resilience in busy, high‑stress situations.
Responsibilities
* Conduct crisis assessments and develop care plans.
* Maintain accurate documentation and engage in positive risk‑taking.
* Act as a gatekeeper to ward admissions when required.
* Collaborate with the multidisciplinary team and share caseload information.
* Use a holistic, bio‑psycho‑social approach to older adults’ care.
* Communicate effectively with service users, families, friends and carers.
* Operate the service’s large geographical area as a qualified driver.
Qualifications
* RMN – Current NMC registration.
* Social Worker/ Occupational Therapist – Current Social Work England / HCPC registration.
* Mentorship experience.
* Practice placement educator.
Experience
* Assessment and management of complex mental health problems.
* Support and supervision of colleagues, students or trainees.
* Use of IT systems for clinical activity record‑keeping.
* Risk assessment and management in varied settings.
* Deliver therapeutic interventions in an integrated multidisciplinary environment.
* Proficiency in NICE, CQC, CQUIN national guidance.
* Holistic care for older adults with physical ill‑health.
* Incidentally, crisis/resolution model experience is desirable.
Physical & Personal Attributes
* Current driving licence and access to a car during the working day.
* Ability to work 24/7 shifts, including on‑call duties.
* Time management, self‑direction, reliability, enthusiasm and flexibility.
* Autonomy, diplomacy, leadership skills.
* Acceptable sickness record over the previous two years.
Training & Special Knowledge
* Assessment, care planning and risk management training.
* Knowledge of Mental Health Act, Health & Safety, Care Programme Approach.
* Clinical Governance principles.
* Audit processes and medicine use (e.g., venepuncture).
* Computer proficiency for clinical systems.
* Equality, diversity and partnership working.
Disclosure and Registration
This post requires a Disclosure and Barring Service check (Rehabilitation of Offenders Act 1975). Applicants must hold current UK professional registration.
Branch and Work Details
Band 6 – Salary £39,959 to £48,117 per annum. Fixed‑term contract of 12 months (full‑time, part‑time or flexible). Working pattern may include 11.5‑hour shifts on average: 3 weeks each month plus 4 shifts in one week for full‑time staff. Base location: Fieldhead Hospital, Wakefield.
