Were currently recruiting a Health & Wellbeing Practitioner to join our dedicated team at HMP Coldingley, a closed Category C male prison in Surrey.
As part of an integrated healthcare framework, youll work alongside a multi-disciplinary team to deliver a mix of clinical and psychosocial interventions to adult male prisoners. This includes individuals presenting with low-level mental health needs and/or substance misuse issues.
You will be responsible for:
1. Managing a caseload and delivering tailored recovery-focused interventions
2. Conducting assessments and co-producing SMART care plans with service users
3. Facilitating structured 1:1 sessions and group programmes
4. Providing harm reduction advice and practical recovery support
5. Supporting release planning and ensuring through-care into community services
