Job Purpose: Mon-Fri 8-6
To work as part of the Children’s Community Nursing Team, taking responsibility for the assessment, management, and evaluation of evidence-based nursing care to children and their families or carers.
To coordinate the day-to-day workload in the absence of Team Leads.
To take responsibility for Learner Nurses on placement with the Team.
Responsibilities:
1. Assess, manage, and evaluate skilled nursing care in negotiation with the child and family in conjunction with the Team Leads.
2. Provide skilled, effective, evidence-based nursing care for children with various needs, such as enteral feeding, intravenous antibiotic administration, and care for technology-dependent children (e.g., tracheostomy care, central venous access).
3. Work collaboratively with other agencies to ensure seamless care delivery to families.
4. Provide information and support to facilitate the child’s and family’s choices regarding nursing care.
5. Promote health education as an integral part of care delivery.
6. Undertake complex discharge planning and assessment, ensuring detailed communication with hospitals and safe discharge for the child and family.
7. Empower parents and carers through education, support, and risk assessment to enable safe community care for their child.
8. Maintain accurate, systematic, and timely clinical records.
