Job Summary

This is an administration role to support the whole team and is full-time of 37.5 hours per week. The Community Crisis Response Team (CCRT) is looking for a Band 3 Co-ordinator to join our team. We support people to stay at home during a time of acute physical illness and offer alternatives to hospital admission. We work closely with partner teams in Derriford Hospital to ensure people are not admitted unnecessarily. The successful candidate will be highly organised, able to work autonomously, and capable of prioritising workload amidst competing demands. You must meet all the essential criteria to apply and be shortlisted.

CCRT is a 7-day service including Bank Holidays. Hours are between 08:00 and 18:00, with some weekends on a rotation basis. This role may not be eligible for sponsorship under the Skilled Worker route; please refer to the Direct Gov website for eligibility information. Livewell Southwest may close the job advertisement earlier than the specified deadline if a high number of applications are received. All Livewell staff are expected to work across a 7-day service.


Main duties of the job

The aim of CCRT is to support clinicians in reviewing and triaging referrals promptly. The successful candidate should be meticulous, capable of working in a team and autonomously with a high level of concentration.

* Based at Mount Gould Hospital; support delivery of person-centred care in home and care home settings across Plymouth.
* Take telephone calls on the enquiry line from GPs, SWAST, health professionals, University Hospital Plymouth, patients, relatives and other external agencies.
* Update dashboards, spreadsheets and data collection tools; complete administrative tasks (e.g., creation of spreadsheets, minute taking, taking messages, IT usage).
* Participate in virtual conferencing; complete and send referrals for planned resources; ensure resources are available for managers to triage and for responders to visit.
* Provide good quality advice and information to customers and professionals within own limitations.
* Document and implement the customer plan, monitor actions, and ensure responses are delivered.
* Raise issues affecting plan implementation with the Responder or Team Leader and act on agreed alternatives.


Communication

Communicate relevant information daily to a range of services/agencies (e.g., GPs, SWAST, specialist nurses, care agencies, customers, families/carers, voluntary agencies). Communications may be by telephone, email, or face-to-face and must be confidential, professional, and compliant with organisational and national policies. Remain flexible to urgent requests to ensure an efficient service.


Organisational

Manage and prioritise workload to meet deadlines in an unpredictable environment. Maintain accurate documentation on required systems (e.g., Eclipse, Care First 6, SystmOne) in line with legislation, policies, and procedures. Report data/telephony/system faults and queries. Collect and enter statistical data; follow health and safety policies and update policy changes as required.


Job Description and Person Specification

Experience (Essential)

* Experience of working with vulnerable people.
* Experience of team working.
* Experience of using IT systems; experience in Excel and Word.

Qualifications (Essential)

* Vocational Qualification Level III or ability to demonstrate the required skills, knowledge and ability.
* Maths & English GCSE Grades 4 and above or equivalent.
* IT qualification (e.g., ECDL) or equivalent.

Knowledge (Essential)

* Understanding of multidisciplinary team working; literate and numerate.
* Evidence of continued personal development; ability to use MS Office (Outlook, Excel, PowerPoint) and various IT systems.

Skills (Essential)

* Good communication skills; able to engage with people across a range of abilities and personalities.
* Strong time management and organisational skills with workload prioritisation.
* Empathic and able to manage emotional situations; flexible to support the team and cope with change.
* Ability to deal sensitively with service users who may have high anxiety or challenging behaviour related to their condition.
