Job overview

The Integrated Community Response Service is a large multidisciplinary team made up of GPs, occupational therapists, a social worker, physiotherapists, nurses, a community matron, and support workers. The main aim of the service is to manage complex clinical cases at home, prevent unnecessary hospital admission and support early discharge of patients following an acute admission. This award-winning rapid response team provides immediate assessment, intervention, early rehabilitation and management for a broad range of patients with complex needs. All members of the team work very closely together and have a good relationship with other community teams and clinicians such as district nurses, GPs, London Ambulance Service and mental health services.

Main duties of the job

We are currently seeking a highly skilled and motivated individual to support the team to manage a complex caseload of patients and provide leadership for the administrative element of the team. Alongside these duties, as part of the senior team, you will be expected to support the junior members of the multidisciplinary team and support the clinical services manager in day-to-day management and service development.The postholder will ensure that all service procedures and processes are effective and safe and enable the service to meet its contract targets and deliver a high-quality patient experience. You must be flexible, organised and have excellent time management skills as the team priorities change frequently throughout the day.

Qualifications

Essential criteria

1. Registered Nurse- NMC Nursing Certificate, DiP, Degree
2. Evidence of professional revalidation
3. Experience of working with/within a multi-disciplinary team
4. Experience of supervision and line management
5. Completion of advanced skills assessment course, Clinical Decision-Making course, Sepsis course, Deteriorating Patient course
6. Independent non-medical prescribing (V300)
7. Knowledge of safeguarding adults/children and completion of mandatory training
8. Practice Supervisor & Assessor
9. Palliative care qualification/proven experience / management of the dying patient
10. Specialist Practice District Nursing or Rapid Response experience

Knowledge

22. Urgent Community Response (UCR) experience or other relevant community nursing experience
