Team Leader

This is an exciting opportunity to join Tudor Court team as a part time Band 5 Team Leader (2 posts available).

Tudor court is a 24 bed Unit within the heart of Heywood. It provides short term non-enhanced residential intermediate care and rehabilitation. The overarching aim of the Unit is to regain and improve the independence of people who might otherwise face unnecessarily prolonged hospital stays or inappropriate admission to Hospital or residential care.

The Unit has both step down and step up function with patients being admitted from their own home. The Care is person centred, focused on rehabilitation and care is delivered by a multi-disciplinary team.


Main duties of the job

As the Team Leader you will be responsible for the day to day management of the Unit and the provision of high quality harm free care to the patient group.

You will manage admissions and discharges to the unit working alongside the Multi Disciplinary Team.

You will be responsible for administering patients medication and monitoring their well-being.

Liaise with other agencies and work collaboratively with partners.

Person Specification


Qualifications

* NVQ Level 3 in Direct Care/Health and Social Care


Knowledge, Training & Experience

* Current experience of working in a health/social care setting
* Experience of working collaboratively within an MDT
* Evidence of continuing development including relevant attendance at courses/conferences
* Experience in assessing patients


Skills & Abilities

* Demonstrate excellent IT skills and experience with health/social care IT systems
* Ability to prioritise and organise own workload and meet deadlines
* Ability to work unsupervised
* Be an effective team member who is able to relate to colleagues at all levels of the Trust with tact and diplomacy
* Ability to coordinate your workload and supervise members of the team, ensuring high quality, safe and effective patient care
* Demonstrate initiative to proactively problem solve with a solution focus
* Ability to use reflective practice, to continuously improve the standard of patient care
