Com Clinical Pharmacist – General Practice
Type: Permanent, Full Time (37.5 hours, some flexibility available)
An established GP surgery in the Reading area is seeking a motivated Clinical Pharmacist with experience in General Practice to join their friendly, multi-disciplinary team. This role is pivotal in delivering safe, effective, and patient-centred Clinical pharmacist services within primary care.
Managing repeat prescriptions, acute prescription requests, medicines reconciliation and transfers of care
Providing structured medication reviews and telephone clinics to support patients with queries about their medicines
Supporting safer prescribing systems, QOF and enhanced services, audits, PINCER, and the Prescribing Quality Scheme
Building strong links with community pharmacies, PCN pharmacy teams, and care homes to improve medicines safety
Must have experience working in a GP surgery
CPPE training completed
