We are seeking an experienced and organised Care Coordinator to join North Birmingham Primary Care Network (PCN), working solely at Shah Zaman Surgery. This part-time role offers 16 hours per week across 3 days. As the lead for all patient recalls, you will play a central role in ensuring patients receive timely and coordinated care, supporting delivery against QOF and the Universal Offer. Experience of using SystemOne is essential, alongside an understanding of recall processes in primary care. Main duties of the job

The Care Coordinator will act as the lead for all patient recalls at Shah Zaman Surgery, ensuring that QOF and Universal Offer requirements are met. This will involve proactive management of recall lists, scheduling, and follow-up to support patients in accessing appropriate care in a timely way. In addition, the post holder will support safeguarding processes, including organising meetings, taking accurate minutes, maintaining the safeguarding register, and working closely with the Safeguarding Lead. They will also assist with GSF coordination and provide administrative support to the Practice Manager. This role requires excellent organisational and communication skills, with the ability to liaise effectively with patients, clinicians, and wider healthcare professionals. Job Purpose
We are looking for an organised and motivated Care Coordinator to join Shah Zaman Surgery under the NHS Network DES. This is a key role at the heart of the practice, with responsibility for leading all patient recall processes. You will ensure patients are invited and supported to attend their reviews in line with QOF and the Universal Offer, making a direct impact on patient outcomes and quality of care.

In addition, you will support the practices safeguarding responsibilities, contribute to Gold Standards Framework (GSF) processes, and provide assistance to the Practice Manager. The role is non-clinical but requires excellent organisational skills, strong communication, and attention to detail. Experience of SystemOne is essential.


Responsibilities by Area

* Patient Recalls (Lead Role): Take full responsibility for the patient recall system within the practice.
* Patient recall management: Manage and monitor recall lists in line with QOF and Universal Offer targets.
* Patient contact: Contact patients proactively, arrange appointments, and ensure follow-up.
* Collaboration: Work with clinical staff to make sure reviews are completed on time.
* Safeguarding: Support safeguarding processes in collaboration with the Safeguarding Lead.
* Meetings and records: Organise safeguarding meetings, prepare agendas, and take accurate minutes.
* Records: Maintain the safeguarding register and update records promptly.
* Escalation: Escalate concerns appropriately in line with policy and statutory guidance.
* GSF: Coordinate GSF meetings and maintain accurate records.
* Registers: Update and manage GSF registers.
* Communication: Support communication between clinicians, patients, and families as needed.
* Practice & Administrative Support: Provide administrative support to the Practice Manager.
* Data quality: Assist with data quality, audits, and reporting linked to QOF and the Universal Offer.
* Service improvement: Support service improvement projects as directed.
* Patient Support & Communication: Act as a key point of contact for patients around recalls and care coordination.
* Navigation: Help patients navigate health and social care services.
* Care plans: Ensure patients understand their care plans and have access to information to support self-care.
* Communication: Communicate clearly, respectfully, and inclusively with patients and carers.


Person Specification

Essential

* Experience working in a GP surgery or primary care setting.
* Knowledge of SystemOne.
* Understanding of QOF and Universal Offer.
* Excellent organisational and administrative skills.
* Strong written and verbal communication.
* Ability to prioritise, multitask, and work independently.
* Commitment to safeguarding, confidentiality, and patient-centred care.

Desirable

* Previous experience in a Care Coordinator or similar role.
* Experience supporting safeguarding or GSF processes.
* Knowledge of NHS primary care structures and the PCN DES.
* Experience contributing to audits and service improvement.
