Job overview
An exciting opportunity has arisen for a Band 7 Macmillan Personalised Stratified Follow Up (PSFU) CNS to cover 12 months maternity leave, starting in October 2024, at Barking, Havering and Redbridge University NHS Trust. The position is full time, covering Monday to Friday, and is an integral part of the Personalised Cancer Care Team (PCC). The PCC team provides personalised care and support for patients living with a cancer diagnosis from diagnosis through their pathway.
You will lead the implementation of PSFU for Breast, Colorectal, and Prostate cancer patients. The service is funded for 2 years by Macmillan, and this 12-month post covers the end of the first year and the start of the second year of funding. The role involves supporting patients to self-manage and gain confidence following treatment, with patient results monitored via a Remote Monitoring System. We also manage a triage line for patient contact post-treatment.
Main duties of the job
The post holder will oversee all aspects of the Remote Monitoring Pathway, act as the expert and superuser for the RMS module of the Somerset Cancer Registry, develop protocols, enroll patients, monitor their progress, and escalate concerns as needed. They will promote RMS uptake, educate staff, and work with MDTSs to identify suitable patients.
