Overview
A fantastic opportunity has arisen for a proactive and motivated Clinical Therapy Lead to join the General Rehabilitation Unit for Community Hospitals at University Hospitals Plymouth NHS Trust. The post holder will be expected to develop, embed and lead the Therapy team in delivering new seven-day rehabilitation services. The post holder will need to be highly experienced in hyper acute / acute rehabilitation and have proven leadership and service improvement skills. Excellent communication skills and a sound understanding of systems working and experience supporting the delivery of rehabilitation across South West Peninsula are critical to this post. The successful applicant will provide essential rehabilitation support and leadership to the wider MDT. Potential candidates must be a HCPC registered Therapist (Dietitian, Occupational Therapist, Physiotherapist or Speech and Language Therapist) with recent experience of leadership, management and service improvement within the acute setting at senior level. Preference will be given to internal Trust staff, as well as 'Priority' and 'At Risk' status including NHS At Risk staff throughout Devon who are able to display recent relevant experience as dictated by the Person Specification.
Main duties of the job
To be responsible for managing and delivering patient centred intensive rehabilitation and reablement within the general rehabilitation units across UHP's footprint inclusive of community hospitals.
* Plan, co-ordinate, deliver and evaluate high quality Therapy service provision to general community hospitals patients and associated clinical areas, with input to future planning for care and management in conjunction with therapy, nursing medical and social care leads.
* Facilitate patient flow, champion a Homefirst ethos and support patients to return home, remain independent for longer and remain at home for longer through engagement with partner organisations.
* Manage a highly complex caseload including patient assessment, diagnosis, treatment and discharge; provide expert resource and specialist advice to staff, patients, carers and other agencies.
* Be a proactive member of the MDT and contribute to service development and training, including audit, general rehabilitation targets, and supervision of junior colleagues within the mixed MDT.
* Support the Therapy Directorate Manager, Community Hospitals Managers and UHP Divisions / Directorates in day-to-day management, coordination and development of clinical service delivery.
Responsibilities (Clinical)
1. To be an expert practitioner, particularly for patients with complex therapy needs.
2. Lead assessment of referred patients requiring therapy and liaise with medical and clinical staff on treatment and care.
3. Provide specialist advice to other therapy staff on management of complex cases and monitor referrals within the acute service.
4. Undertake assessment and care planning of routine and complex patients with respect for privacy, dignity and individuality.
5. Ensure a high standard of patient care by developing, implementing and reviewing clinical standards.
6. Coordinate clinical activity to ensure a responsive service that supports patient flow and referral to other services as required.
7. Identify risks, report accordingly and identify treatment plans to negate risk.
8. Be a core member of the MDT and participate in MDT meetings and ward rounds.
9. Ensure effective two-way communication with patients, carers and other healthcare professionals, supporting equality and diversity.
10. Provide complex information with empathy and reassurance, addressing barriers to understanding where needed.
11. Develop links with clinical specialties across the Trust to promote therapy in disease management, treatment and prevention.
12. Identify opportunities to extend scope of practice and develop integrated working in clinical teams.
13. Comply with local and Trust documentation guidelines; record and submit clinical and professional activity.
14. Lead on aspects of Service Transformation and service improvements as agreed with the Therapy Directorate Manager.
15. Collaborate across the patient’s clinical care pathway with other clinical leads and Therapies Managers to ensure consistency of service delivery.

Person Specification
Knowledge and Experience
* Essential: Varied postgraduate experience at a senior/specialist level as a therapist (OT, Physiotherapy, Dietetics or Speech and Language) within an acute NHS rehabilitation service.
* Essential: CPD portfolio with recent postgraduate courses relevant to general rehabilitation.
* Essential: Extensive clinical experience in specialist rehabilitation including frailty, trauma and orthopaedics, cardiorespiratory, neurological and cognitive difficulties.
* Essential: Advanced skills in assessing and treating motor and sensory difficulties.
* Essential: Knowledge of national guidelines and standards in rehabilitation, home to hospital care and HomeFirst principles.
* Essential: Leadership skills with experience managing people, education programmes and clinical supervision.
* Essential: Research/service improvement skills and recent audit activity.
* Desirable: Prior management experience within an Acute Trust; teaching at postgraduate level; service planning and workload/capacity management; work with commissioners/partners; community and interim care experience; knowledge of amputee and complex spinal conditions; understanding of frailty.
Qualifications
* Essential: MSc/BSc Degree in Therapy (OT/Physiotherapy/Dietitian or Speech and Language).
* Essential: HCPC registration.
* Essential: Ongoing postgraduate training with broad clinical experience in the identified clinical area.
* Desirable: Working towards MSc; Leadership/Management qualification; Service Improvement training; Specialist interest group membership.
Aptitude & Abilities
* Essential: Ability to write and present reports; cope with regular interruptions.
* Desirable: Evidence of ability to question practice and implement change when necessary.
