Role Overview
The Support Worker role sits within the Clinical Assessment Team in the South London Partnership (SLP) Complex Care Programme. This is an exciting time to join the programme as we are expanding our team, working innovatively within rehabilitation, complex care pathways across South London, including the delivery of Personal Health Budgets. A key focus to the SLP Complex Care Programme is to ensure people are supported in the least restrictive setting, close to home and have a personalised outcomes-based support plan that builds on their skills and assets and community connections.
Key Responsibilities
* The post holder will need to have a collaborative approach and have a strong interest in delivering more personalised outcomes for people with complex needs, including via the development of Personal Health Budgets.
* The post holder will play a key role in implementing and evaluating service users' placement support plans in line with their recovery goals, and obtaining feedback from service users and their carers.
* The post holder will undertake some placement visits as lone workers and will work closely with the Clinical Assessors, to support placement stepdown & transitions.
* To work as part of the multi-professional team supporting personalised care planning, implementation and evaluation of commissioned care, in collaboration with the service users, carers and families, and other community/inpatient leads, and including the implementation of Personal Health Budgets.
* To work closely with service users and their carers; to assist service users in their recovery goals which may include supporting them with practical/daily living tasks and engagement in social inclusion activities; working to a high standard and ensuring that as needs change care/support plans are updated.
* To work closely with MDT colleagues, to engage in a range of practical tasks involved in delivering stepdown outcomes for services within the complex care pathways e.g. sourcing providers, making referrals.
About the Programme
The South London Mental Health and Community Partnership (SLMHP) Complex Care Programme (CCP) is a collaboration between Oxleas Foundation Trust, South London and Maudsley NHS Foundation Mental Health Trusts and South West London and St Georges Mental Health Trust. The programme aim is to transform the way decisions are made and how these are used to inform the design of care pathways to achieve improved outcomes for those with complex care mental health needs. A key focus is to ensure people are supported in the least restrictive setting, close to home and have a personalised outcomes-based support plan that builds on their skills and assets and community connections.
From 1st November 2020, the Programme received delegated authority as a Phase 1, to manage the 100% complex care health only funded placement budgets of the SE and SW London Clinical Commissioning Groups (excluding Lambeth). This includes managing complex care placements for c380 individuals at a cost of c£35m. From April 2022, the Programme moved to a Phase 2 receiving delegated authority to agree the health component of the joint funded placements in SWL ICB. This has involved using the Programme's clinical knowledge and lessons learnt from Phase 1 to develop and invest in housing and support initiatives to support people in less restrictive settings and support value for money, and to work more collaboratively with the Local Authorities.
