Are you a dedicated and passionate Social Worker looking to make a positive impact in the lives of adults within our community? West Berkshire Council is seeking a skilled Social Worker to join our Adult Social Care team, reporting to the Team Manager, to provide high-quality social care and support to individuals in need.

The Role

As a Social Worker within the Adult Social Care Service, you will be required:


* To support in the safe discharges of service users from hospital through a joint Health and Social Care approach called Joint Care Pathway.
* To assess or review for eligibility of long term support in accordance with the Care Act 2014 prior to or following their discharge from hospital.
* To work with vulnerable individuals who are unable to make decisions in accordance with the Mental Capacity Act 2005.
* You will promote the rights of individuals and safeguard those who are at risk of abuse by working with the Safeguarding Team and completing S42 enquiries.

The team consists of Occupational Therapists, Social Workers and Social Care Practitioners and we work alongside health colleagues through our Joint Care Pathway. A key role within this team is to develop and maintain professional working relationships with acute and community hospitals which include Basingstoke, Swindon, Reading and West Berks Community Hospital. We have dedicated teams of workers who are assigned to these hospitals to ensure the smooth transition for the service users from hospital to home, or into a care home.

We also have team members who assess service users at home in the community following a period of care under our Joint Care Pathway or following a period of support through Hospital Avoidance.

You will be required to explore all options of support and funding including Continuing Health Care and Fast Track funding. You will attend and contribute to DST's, case conferences and professional meetings, as necessary. You may be required to undertake Court of Protection applications, advise on Lasting Power of Attorney and work with advice from the legal team on matters which require this.

Through Your Role, You Will Be

* Conducting assessments of individuals' social care needs, ensuring a person-centred approach to identify and support their goals and aspirations.
* Providing information, advice, and guidance to individuals and their families regarding available services and support options.
* Collaborating with other professionals and agencies to ensure coordinated care and support for individuals, including health services and community organisations.
* Implementing and reviewing care packages, monitoring progress and making necessary adjustments to meet changing needs.
* Advocating for individuals' rights and ensuring their voices are heard in decision-making processes.
* Working within legal frameworks, such as the Care Act 2014 and Mental Capacity Act 2005, to safeguard and promote the welfare of individuals.

About You

The ideal candidate will possess the following skills and attributes:

* Qualified Social Worker with a recognised social work degree and valid Social Work England registration.
* Experience working in an adult social care setting, with a sound understanding of the challenges and complexities faced by individuals.
* Strong assessment and intervention skills, with the ability to effectively evaluate and manage risks.
* Excellent communication and interpersonal skills, enabling you to build positive relationships with individuals, families, and multidisciplinary teams.
* Knowledge of relevant legislation, policies, and frameworks governing adult social care.
* Ability to work independently, demonstrating initiative and self-motivation, while also being an effective team player.
* Reflective practitioner with a commitment to continuous professional development and learning.
