Job description
We have an exciting opportunity for a passionate and enthusiastic individual to join our dedicated team during an ambitious programme of change within Adults Social Care. The role involves supporting the development and delivery of future goals.
You will be part of the Older Adults and Physical Disabilities team across Westmorland and Furness, supporting people aged 18 and above with long-term care and support needs in the community. Responsibilities include conducting strengths-based assessments, delivering person-centred support, and working collaboratively with health, social care, and third sector organizations to promote independence and community engagement.
