We are looking to recruit a forward thinking, clinical pharmacist interested in developing their career working across sectors in community hospital and primary care improving patient outcomes related to medicines and reducing prescribing variation to ensure patient safety.

You will be motivated, and passionate about patient care, and have excellent communication skills to work with the multi-disciplinary locality team. You will work across health and social services to optimise medications with a patient-centred approach. You will be based mainly in the West area of BCUHB.

Main duties of the job

You will provide clinical pharmaceutical services at community hospitals and in GP surgeries supporting enhanced care and ensuring the application of an evidence based approach to medicines use. The role will also require you to lead on the medicines management agenda in the locality and work with the cluster collaboratives and cluster leads on priority work streams.

You will ideally be a prescriber with the opportunity to develop pharmacist led disease management clinics. Our vision as a team is to be able to offer pharmaceutical care closer to home, particularly focusing on people living with frailty.

The post is open to pharmacists working in all sectors of the profession and could include the opportunity to work across different pharmacy sectors.

On a case by case basis relocation expenses may be available as outlined in the BCUHB policy and can be discussed at interview.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Essential criteria


* MPharm Degree or equivalent
* Post Graduate MSc/Diploma in Clinical Pharmacy or equivalent experience and/or qualifications
* Non-medical prescribing qualification (or working towards)
* Registered with the General Pharmaceutical Council

Desirable criteria

* Accredited or eligible to be accredited as a Higher Education Institute Clinical Pharmacy diploma/MSc tutor
