Job Summary
Bramley, Wortley, Middleton (BWM) PCN is seeking a Clinical Pharmacist to join our well established PCN pharmacy team. This is an exciting opportunity to join an innovative and supportive multi-disciplinary team. The post holder will be a pharmacist, who acts within their professional boundaries, providing clinical support within the PCN and working as part of multi-disciplinary teams in general practice.
The post holder will receive appropriate clinical supervision and ensure practices integrate with community and hospital pharmacy to help utilise the skills mix within teams, improve patient outcomes, ensure better access to healthcare and help manage workload. They will perform a patient facing role to include undertaking clinical medication reviews to proactively manage patients with complex polypharmacy in a variety of settings including the surgery, patient homes and residential care.
Main duties of the job
The post holder will be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy and those with multiple co-morbidities. They will require understanding of the principles of medicines optimisation throughout the PCN and practice teams, promoting good practice in line with national and local requirements.
Responsibilities
* Repeat prescribing and medication reviews
* Ensure patients have appropriate monitoring tests in place when required
* Reduce inappropriate poly-pharmacy and wasteful prescribing and address public health and social needs of patients through clinical medication review
* Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines they need post discharge
* Contribute to reductions in medicine related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines related issues

Person Specification
Essential
* Accredited masters degree of pharmacy Degree (MPharm) (or equivalent)
* Registered with General Pharmaceutical Council (GPhC)
* Completed, or working towards, a Primary Care Pharmacy Education pathway qualification (CPPE)
* Minimum of 2 years post qualification experience
Desirable
* Membership of the Royal Pharmaceutical Society
* Independent prescriber or working towards/intent of gaining an independent prescribing qualification
