St Luke's Hospice is seeking an Integrated Care Facilitator with experience in health and social care, passionate about enhancing the lives of palliative care patients and their significant others.
About the role
You will utilize your specialist knowledge of the health and social care system to support clinical colleagues within the Hospice, primary care, and wider healthcare providers. Your work will involve engaging directly with patients and their significant others in their communities, developing a comprehensive understanding of local initiatives to support them creatively.
You will play a key role in developing and evaluating this innovative role, with your insights helping us improve our services. We recognize the challenges current health and social care provision presents and seek someone who can maximize support and choice from referral to end of life.
The ideal candidate is proactive, team-oriented, and capable of providing support across our three community hubs.
About you
Values are paramount. Experience in clinical settings or specialist palliative care is advantageous but not essential; training will be provided. Knowledge of care provision in Plymouth, South Hams, and West Devon is a plus.
