Overview
Senior Community Nurse role at Sirona care & health in the North and West Bristol Locality. We are looking for a Community Nurse ready to step into a senior position, expand leadership skills and contribute to high-quality, patient-centred care in the community.
Our Integrated Neighbourhood Teams include nurses, therapists, paramedics and support workers who work together to support service users on their healthcare journey. We invite you to join and help us deliver holistic, person-centred care in local communities.
Responsibilities
* Complete holistic assessments of service users.
* Develop and deliver person-centred care plans.
* Line manage junior staff and contribute to their development through supervision, 1:1s and training.
* Undertake and complete the appropriate training, supported by a robust competency framework, to deliver a competent clinical service.
* Communicate with the senior leadership team in the locality to ensure smooth delivery of service.
* Identify risks and act to minimise risk to self, service users and colleagues in line with organisational policy and procedures.
* Coordinate shifts and delegate/allocate care to the team in accordance with policy.
* Act as a point of escalation for staff to report issues or deteriorations.
* Undertake further training as required to develop extended skills and enhance patient care.
