Overview
Urgent Care Services provides short‑term health, therapy and care support to enable service users to remain in the community or home environment, including treating acutely unwell people who would otherwise require hospital admission and supporting hospital discharges.
Responsibilities
* Deliver elements of prescribed packages of care under the guidance and supervision of a Nurse, Therapist or Health Care practitioner.
* Contribute to the ongoing implementation and evaluation of delegated caseloads, reporting all clinical and behavioural outcomes to the delegating nurse or therapist to support re‑assessments.
* Undertake and report on delegated clinical activities that have previously been signed competent, such as baseline vital signs, routine urinalysis, glucose monitoring, wound management, falls‑prevention awareness, equipment provision and activities of daily living to increase independence.
* Actively work as a member of the multi‑professional team to provide high‑quality care to patients.
Qualifications
* Right to work in the United Kingdom and the ability to provide documentary evidence of this status.
* Possession of a full UK driving licence or a full driving licence issued by a EU country (not exchanged from a non‑EU country) if driving is required for the role.
