Specialist Clinical Pharmacist Physical Health

We are looking for an enthusiastic and highly motivated pharmacist to join our friendly pharmacy team within physical health inpatient services in a community hospital setting.
Supported by the Advanced Clinical Pharmacist, you will be responsible for delivering the day-to-day high-quality patient focused clinical pharmacy service on our older people's physical health rehabilitation wards, together with our dedicated and experienced specialist pharmacy technician.
The post holder will be based between the Intermediate Care Unit, Peterborough and Trafford ward, Wisbech, so the ability to work flexibly and travel between sites is essential.
Ideally, we are looking for someone with experience in the care of older people but would consider those who would like to make a move into this specialty and would support you in your development and transition to this role, as well as supporting you to achieve further qualifications including prescribing.
Main duties of the job
The person we are looking for is someone that can positively contribute to the clinical medicines management of older people. The successful candidate will work closely with a multidisciplinary team of passionate and dedicated healthcare professionals to support safe and effective prescribing and use of medicines, and medicines optimisation in these units, supporting patients and carers and promoting independence with medicines.
The post holder will be expected to provide medicines information and advice on all aspects of medicines management to medical, nursing, and other staff on these wards, support the pharmacy technicians and contribute to local education of ward staff.
The post holder will also contribute to clinical audit and assurance monitoring in both ward and community settings, suggesting quality improvements and participating in the development of policies, procedures, and guidelines when required.

Job responsibilities

* Clinically screening and monitoring prescriptions to ensure safe, evidence based and rational use of medicines in line with formulary and policies.
* Participating (in conjunction with pharmacy technicians) in medication history taking and medicines reconciliation for newly admitted patients liaising with secondary care when required to resolve issues.
* Participating in multidisciplinary team meetings and/or ward rounds where possible to provide expertise at the point of prescribing and specialist medicines management advice.
* Supporting prescribing in line with policy, evidence-based practice, following local and national guidelines; and in accordance with local formularies where appropriate.
* Providing specialist medicines information, advice and education to patients, families, and carers to ensure medicines are optimised and improve awareness of medicines and side effects.
* Being proactively involved in the effective discharge of patients and liaison with primary care.
* Promoting independence with medicines and wherever possible by supporting staff and patients with self-administration of medicines, to contribute towards promoting rehabilitation and independence and improve awareness of the use of medicine compliance aids.
* Provide specialist medicines information and advice on all aspects of medicines management to medical, nursing, and other staff.
* Identify the need for written guidance to develop consistent standards of practice regarding medicines management, and participate in the development of policies, procedures, and guidelines.
* Conduct local induction sessions and medicine education for healthcare staff either on a one to one or group basis.
* Support medicines optimisation and provide specialist advice and support to community teams if required.

Person Specification
Edication/Qualifications
* Registered with General Pharmaceutical Council as a practicing pharmacist
* Postgraduate qualification in clinical pharmacy or equivalent
* Independent prescriber
Experience
* Post qualification experience of working as a clinical pharmacist
* Experience of rehabilitation of older people
* Experience of hospital pharmacy and providing a clinical pharmacy service to inpatient wards.
Knowledge & Skills
* Effective communicator by a range of methods with a range of staff and patients
* Knowledge of NHS policy, processes and guidelines relating to pharmacy and medicines management.
* Specialist knowledge of medicines used in older people
Other
* Demonstrates a confident, enthusiastic and positive attitude.
* Ability to travel independently and in a timely manner across Cambridgeshire and Peterborough to meet strict deadlines.
