Frailty and Clinical Triage Practitioner (Nurse or Paramedic) Urgent Community Response (UCR) and Hospital at Home Frailty Virtual Ward Team (H@HVW)
We're looking for a Registered Nurse or Paramedic to join our Urgent Community Response (UCR) team, and Hospital at Home Frailty Virtual Ward Team (H@HVW) delivering rapid, high-quality care to patients in their own homes and preventing hospital admissions where safe to do so.
As a Frailty and Triage Practitioner, you'll work closely with Advanced Clinical Practitioners (ACPs), trainee ACPs, Consultant Practitioners, Physiotherapists, Occupational Therapists, and Health Care Support Workers to provide timely treatment and care planning following initial assessments by senior team members.
In this role you will provide clinical expertise, by providing assessment, advice, and information according to clinical guidance through telephone triage, and face-to-face assessments. You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards.
During triage shifts you will be the overall shift co-ordinator ensuring all patients are seen in a timely manner to enable high quality, compassionate care while process telephone referrals.
The post holder will be expected to work flexibly between clinical triage and patient facing shifts within our service hours of 8-8, including 1 in 4 weekends.
Main duties of the job
The post holder will provide clinical expertise, by providing assessment, advice, and information according to clinical guidance through telephone triage and face to face assessment.
*To be a central contact for the UCR team, using initiative to triage phone calls and emails ensuring appropriate timely action is taken.
*Work closely with the Triage Support, using judgement and experience to decide which calls and documents can be delegated to them.
*Support the coordination on the daily team handover
*Coordination of appointments within a very busy team
*They will provide assessment and care planning including history taking, physical assessment and treatment planning, without direct supervision
* Face to face assessment and treatments of frail patients who are acutely unwell provide hospital level care at home whenever safe to do so.
