PCN Clinical Pharmacist - South East Shropshire PCN
We are seeking a full time Clinical Pharmacist to work across practices within the South East Shropshire Primary Care Network (SES PCN).
Please note that the PCN would consider candidates that wish to work 30 hours or more a week over 4 to 5 days.
We are ideally looking for Clinical Pharmacists who have already completed the CPPE Pathway.
This is an exciting opportunity to join our existing team. Professional Leadership will be provided by the Consultant Pharmacist at OHP and the Lead Clinical Pharmacist in the PCN.
Main duties of the job
The post holder is a clinical pharmacist, who acts withintheir professional boundaries, supporting and working alongside a team ofHealth Care Practitioners within general practice and a Primary Care Network(PCN).

Job responsibilities
The post holder will provide primary support to generalpractice staff with regards to prescription and medication queries. They willperform medicines reconciliation on transfer of care and implement systems forsafer prescribing, providing expertise in clinical medicines advice whileaddressing both public and social care needs of patients in the PCN.
The post holder will work as part of a multi-disciplinaryteam in a patient-facing role. They will take responsibility for areas ofmedicines optimisation within the PCN and undertake structured clinicalmedication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide clinical leadership onmedicines optimisation and quality improvement (QI) and manage some aspects ofthe quality and outcomes framework and enhanced services at practice and PCNlevel.
The post holder will work with all sectors of pharmacyacross the health system and the wider OHP organisation to improve populationhealth. Demonstrating improvement in patient outcomes and working to reducehealth inequalities across the PCN contributing to improving the efficiency ofgeneral practice and delivering a best in class service.
The post holder will be supported to develop their role inline with the OHP workforce strategy, this will include becoming and operatingas a non-medical prescriber. This is a new role and will be reviewed at leastannually, where the Job Description may be updated in line with the needs ofthe organisation.
