This part time role (30 hours per week) is for Medicine Management Optimisation across the disciplines of both General Practice and Care Homes. If you are interested in working in this dynamic environment then please come and join this enthusiastic dynamic team. As a Clinical Pharmacist in Surrey Heath you will join the growing clinical pharmacy and multi-disciplinary PCN team. Working across a nominated general practice to support the responsibilities and deliverable of the Surrey Heath Primary Care Network. Initially, your time will be allocated on rotation across one nominated GP practice and also to the PCN Care Homes Team, to enable you to build your clinical skills and confidence and benefit from the diversity of portfolio working.
Responsibilities
* Support the practice(s) to implement effective medicines management, identify areas for improvement, and initiate and manage change.
* Support the Primary Care Network to implement the medicines optimisation requirements of the new Network Contract, Direct Enhanced Service for enhanced health in care homes and other PCN medicines management responsibilities.
* Undertake resident/patient-facing activities and associated medicines management support to ensure effective and efficient use of medicines within the practices and across registered populations.
* Work on rotation across one nominated GP practice and the PCN Care Homes Team to develop clinical skills and portfolio diversity.
Training and Requirements
This is a training role which requires the successful applicant to complete a CPPE course; please ensure you understand the training requirements.
About the Organization
These roles have been designed to be interesting and fulfilling roles, providing a comprehensive range of appropriate clinical pharmacy services to general practice in Surrey Heath. The post holders will be supported to help develop the role and work to improve the care and outcomes for the patients across Surrey Heath PCN. We are Surrey Heath Community Providers Limited, a federation of 7 GP practices across 10 sites, covering a population of over 97,000 patients across Surrey Heath. We began in 2016 and now employ around 200 members of staff across our clinical and corporate support teams. Since 2019 we have hosted the Primary Care Network in Surrey Heath. As a GP Federation and Primary Care Network, we are proud to represent our member practices and to champion primary care by working with local general practice and system partners, in the provision of community based healthcare services.

Salary
£47,758 to £54,651 a year Depending on experience (Pro rata for part time)
