The Role:
Join our Reablement Team as a Community Reablement Assistant (Level 2) and support adults over 18 in Hampshire needing extra care. Our goal is to maximise their independence and delay or avoid ongoing care, helping them stay at home. Our team, including Community Reablement Assistants, Occupational Therapists and Sensory Officers, collaborate with Health colleagues to support those with short or long-term conditions and those transitioning from hospital.
What you’ll do:
* Provide Person-Centred Care: Conduct initial home visits and independently perform reablement interventions to restore independence including intimate personal care, continence care, and moving and handling.
* Deliver Outcomes: Perform risk assessments, set goals, update reablement plans for Level 1 Assistants and monitor service user progress.
* Support Operational Delivery: Supervise and coach Community Reablement Assistants and assist Team Leaders, Occupational Therapists and Sensory Officers with activities.

* Knowledge: Understanding of the needs of service users, particularly in relation to reablement programmes.
* Experience: Previously worked within a social or health care setting.
* Values: Role models our core values and demonstrates patience, compassion, empathy and excellence in all interactions. You’ll treat all service users with dignity and respect.
* Relationship Management: Builds successful working relationships through strong interpersonal and communication skills, with a willingness to seek advice from appropriate professionals.
* IT skills: MO365 experience (including Word, Outlook and Excel).

Working Patterns
Our Service runs 07:00-22:00, 7 days a week, all year-round, therefore you’ll be required to work evenings and/or public holidays and/or weekends as part of your Normal Working Hours, for which enhancements are paid in line with the HCC salary policy. Details of the working patterns available will be discussed in more detail during the selection process.

Travel Requirements
You will be required to travel to various locations within Hampshire and it is essential that you have a current drivers’ licence and access to a reliable vehicle on a regular basis.
