Job summary
A fantastic opportunity has come up for a dynamic and enthusiastic qualified nurse to join the Stockport integrated diabetes specialist service. The post offers the right individual the opportunity to work within all elements of the specialty and to gain experience working alongside a multiprofessional team that supports the individualised holistic care of patients with diabetes. You must be a qualified nurse with relevant experience in diabetes, either in a specialist diabetes role or in a role where you can demonstrate provision of enhanced diabetes care. Ideally you will have undertaken an accredited course in diabetes and be able to demonstrate knowledge of relevant diabetes guidelines. You will also hold a teaching and/or mentoring qualification and have experience in education, teaching and mentorship for health care professionals and patients. You will be able to work autonomously as well as as part of a multidisciplinary team, manage your own workload, undertake nurse-led clinics, multidisciplinary clinics and home visits as required, and play a key role in supporting and educating patients and healthcare professionals to improve the quality of diabetes care."
Main duties
Be professionally and legally responsible and accountable for all aspects of own work using evidence-based practice and expert knowledge in diabetes to influence the management of patients in your care (in accordance with national and local Trust policies and NMC guidelines).
Be clinically responsible for a designated caseload of patients with diabetes, ensuring high standards of care. Be prepared to undertake training to fulfil further duties identified as part of service developments or performance review.
Organise workload effectively and efficiently with regard to clinical priorities and time use, in accordance with service demand and requirements on a changing basis.
Provide telephone assessment and education to patients, relatives/carers, including counselling and goal setting on risk factor modification, addressing barriers such as language or education to facilitate appropriate care on the phone.
Act as an educational resource for patients, families/carers and other healthcare professionals, with respect to all aspects of diabetes, its signs, symptoms and management.

Education & Qualifications Essential
* 1st level registered Nurse - NMC
* Nursing degree/diploma level or equivalent experience
Knowledge Essential
* Substantial post-registration experience at band 5 or above in the field of diabetes
* Knowledge of guidelines and quality frameworks for diabetes
* Demonstrate initiative and innovative practice
Desirable
* Knowledge of diabetes specialist clinical treatments and technology
Experience Essential
* Ability to work autonomously and within a team
