Supporting service users with activities of daily living- personal care needs, meal preparation, medication and domestic tasks where required. Providing practical and emotional support and guidance to service users Providing reactive, focused, and flexible support to adults Responding to referrals from Clinicians and Community Health Professionals Assess service users' needs, complete and follow a support plan for the individual. To be a successful Support Worker, you'll: Have the Right to Work in the UK, unfortunately we can not offer sponsorship for this role.
