Pay: £20 per hour (via umbrella company) (£15 PAYE an hour)
About the Role Gloucestershire County Council is seeking two Rehabilitation Services Coordinators to join our dedicated, multi-disciplinary team at Great Western Court in Gloucester. Our centre provides comprehensive rehabilitation and 24-hour care for individuals following a hospital discharge or admission from the community — supporting them to regain independence and live fulfilling, dignified lives.
In this rewarding role, you will play a key part in coordinating personalised rehabilitation programs, leading shifts, and supporting a team of Rehabilitation Assistants to deliver exceptional care.
Key Responsibilities
Lead shifts and coordinate rehabilitation and care programmes tailored to individual needs.
Safely administer, prompt, or assist with medication in line with care plans.
Support residents with mobility, personal care, and daily living tasks.
Collaborate with the therapy and nursing teams to develop, review, and implement care plans and risk assessments.
Maintain accurate written and electronic records.
Facilitate smooth admissions and discharges, promoting recovery and independence.
Create a positive, caring, and stimulating environment for all residents.About You We’re looking for someone with:
Experience in a rehabilitation, care, or health support setting.
Strong leadership skills and confidence in leading shifts.
Excellent communication and teamwork abilities.
A compassionate, patient, and proactive approach to supporting individuals.
