We are recruiting a Community Mental Health Support Worker to join our Primary Care Network (PCN) team in South Tyneside, working as part of the Enhanced Primary Care Hub based at the CNTW NHS Bedewell Unit in Hebburn. This is a permanent, part-time role, contracted at 22.5 hours week. This is a hybrid role. This role is not eligible for sponsorship as it does not meet the requirements as set out by the UK Visa and Immigration Service.
About the role
In this role, you will provide one-to-one, recovery-focused support to adults living in South Tyneside who are experiencing mental health challenges. You will use your skills in active listening, empathy, and person-centred practice to help individuals identify their needs, develop goals, and work towards meaningful change. Support is delivered predominantly face-to-face at Bedewell, with limited telephone contact. Sessions are typically weekly in the first month and then move to fortnightly depending on individual need. You will play a key role in supporting people to maintain hope, build resilience, and improve their overall wellbeing, while ensuring services are delivered in line with our values of compassion, accountability, respect, excellence, and innovation.
What you'll do

* Help people develop coping strategies, build resilience, and maintain hope.
* Support engagement in community activities and the development of social networks.
* Promote independence and informed decision-making.
* Signpost and connect individuals to appropriate services and community resources.
* Use a range of communication methods (face-to-face, phone, text, email).
* Develop and maintain knowledge of local pathways and support options.
* Support the development of sustainable peer and community support networks.
* Manage caseloads effectively, ensuring accurate and timely record keeping.
* Work in a culturally sensitive, inclusive way, challenging stigma and discrimination.
* Support a diverse range of service users from varied backgrounds and communities.
* Participate in supervision, reflective practice, and ongoing professional development.

* Experience within health, social care, or VCSE settings.
* Understanding of mental health, recovery, and the impact of stigma.
* Strong communication and interpersonal skills.
* Ability to build effective working relationships at all levels.

* Good organisational and time management skills.
* Ability to manage caseloads and work independently and as part of a team.
