Overview
Surrey Downs Health & Care are seeking a dedicated Dietitian, whether you are newly qualified or have experience. You will collaborate with our Lead Dietitian and team members to provide dietary services across settings including patients\' homes, community hospitals, care homes and clinics. The role offers exposure to a range of conditions from nutrition support to managing coeliac disease, gastroenterology issues, weight management, diabetes and more. We offer a hybrid working model with flexibility to work remotely or at multiple sites across the borough, with opportunities for student training and professional development including regular supervision.

Responsibilities
* Work as part of the team undertaking lead responsibilities as negotiated and agreed with District Nurse.
* Treat all patients as individuals, respecting their privacy and dignity at all times.
* Assess individual care needs, implement and evaluate holistic programmes of care.
* Involve, support, inform and educate family/carers.
* Promote health and wellbeing of the patient wherever possible.
* Develop effective communication and liaison systems.
* Maintain clear, comprehensive, signed and contemporaneous records according to SDHC procedures.
* Facilitate/undertake training and professional updating for team and students.
* Act as an assessor and mentor to qualified and unqualified staff.
* Maintain, develop and monitor standards of evidence-based care.
* Maintain own professional and clinical integrity in line with NMC guidelines.
* Support/mentor team members and participate in clinical supervision activities.
* Work without direct supervision and liaise with the senior team member, deputising as required.
* Be responsible for timely patient care and data entry/record keeping.
* Work in cooperation with other professionals and agencies involved in patient care.
* Be aware of and act upon procedures that protect vulnerable individuals when necessary.
* Participate in the SDHC Clinical Governance Strategy and Risk Management Strategy.
* Undertake other duties consistent with the post as required; staff may be asked to move base temporarily or permanently as service needs.
