Overview
We have an exciting opportunity in the Cumberland 0 - 19 Service working within our 0 - 19 Team for a Child Development Practitioner. The successful applicant will be based in Whitehaven, working within our teams to deliver care across the localities. The Child Development Practitioner is a member of the 0-19 Public Health Nursing Service, working within the policies and procedures relating to the 0 - 19 service. The post holder will work closely with the SCPHN within a skill-mixed team to support the delivery of the 0-19 Service Specification and Healthy Child Programme (HCP). The aim of the service is to provide universal access to children, young people and families to the Healthy Child Programme and facilitate early identification of additional and/or complex needs. The post holder will contribute to the Public Health Agenda, to support the promotion of health and reduction in health inequalities. They will work in partnership with key partners and agencies to meet local and national agendas.

Responsibilities
* Support the delivery of the 0-19 Service Specification and Healthy Child Programme (HCP) within a 0-19 Public Health Nursing Service.
* Work in partnership with the SCPHN in a skill-mixed team to provide universal access to the Healthy Child Programme for children, young people and families.
* Contribute to identifying additional and/or complex needs at an early stage and support appropriate referrals and care pathways.
* Promote health and reduce health inequalities as part of the Public Health Agenda.
* Collaborate with key partners and agencies to meet local and national health objectives.
