Job Summary
We are seeking a dedicated and knowledgeable Trackman. The ideal candidate will play a crucial role in providing various works on track. The Trackman will be responsible for any works needed be the site manager or any authority on site.
Responsibilities
* Must have PTS and a CSCS Card, works will be on drainage.
* Conduct discharge planning to facilitate smooth transitions from inpatient to home or community care.
* Provide direct patient care, ensuring comfort and safety while addressing physical and emotional needs.
* Utilize knowledge of physiology, anatomy, and medical terminology to assess patient conditions effectively.
* Assist in physiotherapy sessions, including trigger point therapy and orthopedic rehabilitation.
* Maintain accurate documentation using EMR systems to track patient progress and treatment outcomes.
* Engage with pediatric, geriatric, and other specialized populations to deliver tailored healthcare services.
* Support patients in both inpatient hospital settings and outpatient clinic environments.
Qualifications
* Strong understanding of discharge planning processes and acute care protocols.
* Experience in developing comprehensive care plans for diverse patient populations.
* Background in pediatrics, geriatrics, orthopedics, or home & community care is preferred.
* Proficiency in medical terminology and familiarity with EMR systems is essential.
* Previous experience in hospital or clinic settings is highly desirable.
* Knowledge of physiology and anatomy to provide effective patient assessments.
* Ability to work collaboratively within a multidisciplinary team focused on delivering high-quality patient care.
