Advanced Occupational Therapist – Bradford District Care NHS Foundation Trust
Bradford District Care NHS Foundation Trust is strengthening how we deliver its Mental Health Rehabilitation Services to reduce health inequalities and improve outcomes for individuals with severe mental illness (SMI). The Advanced Occupational Therapist will perform advanced therapy role functions and clinical leadership in a specialist pilot team, proactively developing processes to identify, engage and facilitate recovery, rehabilitation, and habilitation of service users stuck within acute inpatient beds or inappropriate residential placements, and face cycles of re‑admission from community placements.

Responsibilities
* Maintain and develop core clinical skills to deliver holistic functional assessment and evidence‑based interventions.
* Support admission avoidance or timely step‑down into varied settings including NHS rehab, Hollingwood Lane rehab supported housing, wider supported housing, and independent community settings.
* Collaborate closely with multi‑organisational teams—care coordinators, clinical leads, peer support workers, rehabilitation programme manager—to develop referral pathways and a ‘no wrong door’ approach.
* Recognise opportunities for wider service and pathway development to meet NG 181 and GIRFT recommendations.
* Take a proactive role in service improvement, underpinned by the Trust’s core values.
