Mediplacements is pleased to be working with a GP practice / Primary Care Network (PCN) in Hertfordshire to recruit an experienced Primary Care Pharmacist for a 6:month fixed term contract.

This role is ideal for a pharmacist with proven experience in Atrial Fibrillation (AF) reviews, QOF, and Enhanced Care Framework (ECF) delivery, looking to make a real impact within primary care.
The Role
You will support the practice/PCN by delivering high:quality clinical pharmacy services, with a particular focus on AF management and anticoagulation safety. Working closely with GPs and the wider multidisciplinary team, you will help improve patient outcomes while supporting the delivery of key contractual targets.
Key Responsibilities:
Deliver structured Atrial Fibrillation (AF) reviews in line with clinical guidance
Review and optimise anticoagulation therapy, including DOACs
Support the achievement of QOF and Enhanced Care Framework (ECF) requirements
Identify patients requiring intervention to improve safety and outcomes
Provide medicines optimisation support and clinical advice to the wider team
Maintain accurate clinical records and coding
Communicate effectively with patients and healthcare professionals
Essential Requirements:
MPharm degree or equivalent
Current GPhC registration
Demonstrable experience delivering AF reviews in primary care
Strong understanding of QOF and ECF frameworks
Experience managing anticoagulation, including DOACs
Previous experience within a GP practice or PCN
Excellent communication and consultation skills
Desirable:
Independent Prescriber qualification
Experience using EMIS or SystmOne
