Salary: Hospice Clinical Band 7 £44,835 to £51,307 (pro rata for part-time)

Join Our New 24/7 SPUR Service as a Clinical Specialist Triage & Facilitator Nurse
At Birmingham Hospice, our patients are at the heart of everything we do. We are launching a new 24/7 Specialist Palliative Urgent Response (SPUR) service, operating between 8:00 PM and 8:00 AM to respond to urgent palliative care needs across Birmingham and Solihull.
The role will be based at the first point of contact for all incoming calls to the service, providing clinical leadership and autonomous practice in collaboration with other healthcare professionals to assess, diagnose and treat patients in their place of residence, ensuring timely clinical responses.
Responsibilities
* Act as an autonomous practitioner working independently and in conjunction with other health professionals.
* Assess, diagnose and treat conditions of patients within their own homes; initiate referrals as appropriate.
* Independently prescribe appropriate medication and evaluate or refer to other specialists as necessary.
* Collaborate with hospice, community, primary care and acute teams to deliver clinical leadership and ensure adherence to policy and procedures.
* Provide expert professional advice to patients, carers and colleagues; maintain clinical excellence.
* Coordinate staffing resources with heads of service to deliver priorities.
* Provide education and training to other staff and students.
* Prioritise and triage patients with palliative, End-of-Life and acute conditions; refer as appropriate.
* Implement and evaluate personalised care planning and advanced care planning in accordance with evidence and guidelines (NICE/NSF).
* Be sensitive to changing physical, psychological and spiritual needs of patients and families.
* Maintain accurate clinical records in line with current legislation; manage safeguarding as required.
* Support patients with prescribed medicines, including non-prescription medicines within scope of practice.
* Liaise with external services and community teams to ensure continuity of care.
* Lead and participate in shared learning; contribute to service development and quality improvement.
Qualifications
* Registered Nurse (Adult) qualification (e.g., RGN) and NMC Registration
* Independent prescribing qualification (V300) or equivalent specialist training
* European Certificate in Essential Palliative Care (or working towards)
* Advanced communication skills training
Desirable/Experience
* Masters degree (Healthcare related) or working towards Level 7
* Teaching qualification or demonstrable experience
* Experience in end-of-life care, triage, and independent practice in palliative care
* Knowledge of NHS policies, patient involvement, service development, mentoring and leadership
* Budgetary management, change management, audit and evaluation experience

* Ability to communicate highly complex information to diverse audiences
