Dr Manickam & Partners is a relatively small (circa 4000 patients) but very friendly practice based within the Hednesford Valley Health Centre.
The working hours are currently 14 per week spread over three days of the week.
Main duties of the job
* Deliver practice nursing services as part of the multidisciplinary team, providing care within scope of practice to the entitled patient population.
* Responsible for infection prevention and control, health promotion, health prevention, and actioning test results in accordance with policy.
* Support the management team in reviewing clinical policy and procedures.
* Work within patient group directions where these are available and maintain required level of training and competence.
* Wear a name badge and uniform at all times, supplied by the Practice.
* Carry out childhood immunisations, cervical cytology, ECG, health promotion, chronic disease management (including diabetes care), contraception and sexual health advice, weight management.
* Provide simple wound care and removal of sutures; chaperone other clinicians when required.
* Assess, plan, develop, implement and evaluate treatment programmes that promote health and well‑being.
* Proactively identify, diagnose and manage treatment plans for patients at risk of developing long‑term conditions.
* Work with other health professionals to diagnose, monitor, manage and treat long‑term conditions.
* Assist patients to adhere to prescribed treatments and adopt health promotion strategies.
* Implement and participate in vaccination and immunisation programmes for adults and children.
* Carry out new patient checks and lead on infection control measures within the Practice.
* Maintain GP and Nurses rooms, stock and rotate items as required, keeping the cold chain and fridge temperatures.
* Undertake housekeeping duties: general tidiness and cleanliness of nurses room.
* Order health promotion literature and stock.
* Action laboratory results in accordance with practice protocol.
* Participate in administrative systems and attend Practice meetings when required.
* Undertake home visits as required (e.g., for vaccinations).
* Perform any other delegated duties appropriate to the post.
Qualifications
* General Practice Nurse qualification.
* Knowledge of Quality Outcome Framework and other NHS income streams.
* IT literacy.
* Disclosure and Barring Service check.
* UK registration and current professional registration.
* GMC requirement: if English is not first language, completed recognised qualification regarding fluency in English.
