Job Overview
Registered General Nurse / Registered Mental Health Nurse, Grade NHS AfC Band 6, contract permanent, 37.5 hours per week (up to 37.5). Working hours 8:00–18:00 on rota basis.

Responsibilities
* Provide professional leadership and support to the Enhanced Care team, working with the Clinical Lead and team coordinator.
* Conduct holistic clinical assessments and support 1‑1 enhanced care for patients requiring higher levels of supervision.
* Allocate Enhanced Care Support Workers to wards and coordinate enhanced care interventions in partnership with ward teams and families/carers.
* Lead and support Quality Improvement projects across the trust, demonstrating up‑to‑date practices and compliance with national and local guidelines.
* Organise and deliver trust‑wide training in line with the Dementia Strategy, Butterfly Scheme, and John’s Campaign.
* Collaborate with the Dementia Lead Matron to ensure ongoing staff development and maintain appropriate training and competency records.
* Assist in recruitment, selection and mentorship of junior staff and Enhance Care Support Workers.
* Adopt and promote evidence‑based interventions, maintain accurate clinical and administrative records, and support safe patient care and risk planning.
* Act as a patient advocate and promote shared decision‑making across the patient pathway.
* Represent the trust in external voluntary sector and community partnerships where relevant.
Qualifications and Training
* Diploma or Degree in a relevant field of practice (NMC/HCPC registration required).
* Teaching/Mentoring Certificate or equivalent skills.
* Evidence of Continuing Professional Development in a specialist field of practice.
* Post‑registration experience with people with dementia, delirium or cognitive impairment.
* Up‑to‑date professional knowledge and understanding of national and local guidance.
Knowledge, Experience and Expertise
* Substantial experience as a Band 5 nurse or equivalent.
* Knowledge of national dementia and delirium guidance and strategies.
* Demonstrated understanding of current issues in health and social care.
* Experienced in effective patient and public involvement.
* Evidence of recent education in relevant field.
Health & General
* Able to fulfil the health requirements of the post and undertake any reasonable adjustments.
* Eligible to work in the UK and able to work across the Calderdale and Huddersfield network.
