Primary Care Mental Health Worker

The Hertfordshire Partnership NHS Foundation Trust is transforming Adult Community Services and delivering a new Enhanced Primary Care Mental Health Service to people in Watford, Hemel Hempstead and St Alban's. This role is an exciting opportunity to work in a developing service, using needs‑led pathways and continuous quality improvement to shape the service.
Key Responsibilities
* Build trusting and collaborative relationships with service users and carers.
* Assist in the formulation, implementation and monitoring of recovery care plans and attend multi‑disciplinary meetings.
* Provide low‑intensity psychological interventions to support service users in achieving personal goals and resolving concerns with anxiety and low mood.
* Act as an effective link between General Practitioners, housing, carers centre, statutory and non‑statutory agencies and secondary care, providing advice and developing joint working relationships.
* Support the recovery journey of patients discharged from secondary care to primary care during the transitional phase.
* Identify and support GPs in the management of patients who are frequent users of services.
* Maintain up‑to‑date knowledge of local community resources to support service users’ recovery goals.
* Work flexibly to meet identified patient need, including advocacy for access to benefits and accompanying patients to facilities if necessary.
Person Specification
* Experience: Ability to manage a workload independently; experience working with service users in the community and people with mental health or complex needs; experience of mental health services (as a worker or service user/carer).
* Training and Qualifications: Completion of the Care Certificate or equivalent; NVQ Level 3 in Mental Health with post‑qualifications at diploma level or NVQ 4; relevant healthcare training for people with mental health problems; GCSE level education (minimum 5 A–E qualifications including Maths and English); relevant healthcare professional diploma/degree.
* Knowledge: Resources and working practices across health and social care; Mental Health Act 1983 (and amendments) and Mental Capacity Act 2005; safeguarding of children and adults.
* Other: Full valid driving licence and access to a car for business purposes (unless covered by the Equality Act 2010 disability provisions).
